Hospital to Home Packs
The Redcliffe Hospital Foundation is proud to be involved with the Rotary Club of Redcliffe Sunrise in this worthwhile project.
The H2H project, an initiative of Redcliffe Sunrise Rotary Club, aim to address the problem of the elderly and frail face when returning home from hospital. Not being able to have sufficient care at their home, many are readmitted to hospital.
Hospital to Home Packs seek to address this problem by providing essentials to assist in the care of elderly people after they leave hospital.
Endeavouring to prevent readmission by ensuring their basic needs are met in their own home allows valuable time for other community agencies to commence service, such as Meal on Wheels. In the interim, Hospital to Home Packs can be a lifesaver.
